Purpose

1. Ask what the form is for

A camp form may support screening, referral, billing, public-health reporting, admin, or marketing. Those are not the same thing.

  • Who is organizing the camp?
  • Which clinic, lab, employer, community group, or vendor is involved?
  • Which fields are required and which are optional?

2. Ask who sees the information

The useful question is not only what you write down. It is who receives it after the queue moves.

  • Will the desk, clinician, lab, organizer, employer workflow, app vendor, or public-health team receive this?
  • Will any ID or health-ID field be used for follow-up?
  • Where will the record go after the camp?
Follow-up

3. Ask who explains results

A result without an owner can become confusion, sales pressure, or silence.

  • Who explains abnormal, borderline, or unclear results?
  • Is follow-up clinical, administrative, sales-driven, or public-health related?
  • How will I know who to contact later?

4. Keep a simple private after-camp note

You do not need a dossier. A local note can record the camp type, date, follow-up owner, and one question for qualified care.
